Quilt Order Form
Important Instructions:
Complete this form and press the submit button at the bottom of the page:
Please print 2 copies of the completed order form (the page that appears after you press submit), one for your records and one to include with your shipment of t-shirts.
You will receive an email within 24 hours from us with a final draft of your order. At this time we will also request complete payment information (for credit card purchases).
Shipping:
Place shirts in a plastic bag before placing in a box. If you are including photo transfers in your order please include the original document (photo, invitation, certificate, etc) in the shipment. Please ship using US Postal Service, UPS, or FedEx.

Confirmation:
As soon as we receive your shipment of t-shirts we will confirm your order by e-mail and give you an approximate date when you will receive your quilt. Feel free to contact us by email or phone at any time with questions or concerns.
Payment:
We will contact you regarding final payment when your quilt is completed.
Delivery:
Allow 4-6 weeks for delivery. If you have a specific date when you need the quilt, let us know at time of order in the special instructions box on the order form.
